Healthcare is a defensive sector that pays relatively modest dividends, and the combination makes it a common core holding for income portfolios rather than a source of headline yields.
Demand for medical care does not fall much in a recession, which gives the sector revenue stability comparable to consumer staples. But healthcare companies also spend heavily on research, and that spending competes directly with the dividend for the same cash. A large pharmaceutical company typically commits a substantial share of revenue to a drug pipeline whose output is uncertain — which is why yields here average around two percent rather than the four or five common in utilities or energy.
The sector's specific risk is patent expiry. A drug that loses exclusivity can lose most of its revenue within a year or two as generics enter, and a company whose earnings are concentrated in one or two products faces a step change rather than a gradual decline. When assessing a healthcare dividend, the relevant question is usually less the current payout ratio than what the earnings base looks like once the next major patent expires.
Several of the largest names hold multi-decade records of consecutive dividend increases, which is a meaningful signal in a sector where earnings can shift abruptly.

Includes US-listed companies classified in the Healthcare sector that currently pay a dividend, ranked by trailing twelve-month yield. The classification covers pharmaceuticals, medical devices, providers and insurers. Exchange-traded funds and mutual funds are excluded.
